VERIFIKASI PENDATAAN PTK DIGITAL 2013

Assalamualaikum Wr.Wb.
Mengingat pentingnya data PTK digital yang harus diisi oleh guru Madrasah/RA & Guru PAI di Lingkungan Kantor Kementerian Agama Kabupaten Malang dalam rangka untuk perhitungan kebutuhan anggaran, maka semua guru diharap untuk melakukan pengecekan / verifikasi data PTK Digital 2013 baik guru yang sudah lulus sertifikasi dan yang belum sertifikasi.
Mekanisme Verifikasi data :
  1. Download PTK digital hasil verifikasi GURU MADRASAH & GURU PAI
  2. KKM / KKG memverifikasi apakah data guru dari kecamatan masing-masing sudah ada/tidak, benar/salah
  3. Jika data sudah ada &  benar maka tidak perlu mengumpulkan PTK digital
  4. Jika data tidak ada / salah, maka HARUS mengumpulkan PTK Digital. Formulir dan tata cara pengisian PTK digital dapat di download disini !!!!
  5. Bagi guru yang lulus sertifikasi tahun 2012 dan telah menerima sertifikat pendidik, dimana status data PTK digital tertulis SEDANG SERTIFIKASI / BELUM SERTIFIKASI maka data di PTK digital harus diganti dengan status LULUS SERTIFIKASI (semua data harus ditulis sesuai dengan sertifikat pendidik, kecuali NRG)
  6. Format PTK digital yang benar di save dalam bentuk document (.docx) dengan nama NUPTK guru ybs. misal : 1234567891234567.docx, penyimpanan PTK digital yang salah akan berakibat pada tidak terbacanya file tersebut dalam aplikasi.
  7. Data dikumpulkan di KKM untuk guru Madrasah/RA, dan KKG untuk guru PAI di kecamatan masing-masing.
  8. Data dikumpulkan oleh KKM & KKG ke MAPENDA paling lambat hari Senin, tanggal 18 Maret 2013, jam 14.00 WIB
  9. Diharap semua guru Madrasah/RA dan Guru PAI yang mempunyai NUPTK baik  LULUS SERTIFIKASI, SEDANG SERTIFIKASI dan YANG BELUM SERTIFIKASI mengumpulkan PTK digital.
